This next part Is the most important part. I will need three samples of your hair. One from the front area, one from the top area and one from the back area of your head. I need two hair strands from each of these areas, they must be snipped close to your scalp and must include any new growth. If there is an area that is of particular concern or any areas that your hair is coming out from, please include and describe. Please also include the date of last relaxer, type of relaxer ( lye or no lye or other ), brand if possible, any other chemical service(s) within the last year, other hair services such as braids or weaves and all the products you are currently using on your hair.
